Required Documents for Claim:
Claim form filled in by the beneficiary
A copy of ID card/Passport/Birth Certificate of the Insured;
Supporting document of beneficiary (family book, residential book, Loan agreement);
Police report of accident
Medical documents and reports related to treatment;
Recent photo of the Insured;
Death certificate;
Letter of loan offer issued by lender (payment schedule etc)
Proof of disablement provided by licensed Hospital/Clinics

Requirement document for Death claims:
Completed claims form by the beneficiary
A copy of ID card/Passport/Birth Certificate of the Insured;
Document proof of beneficiaries;
Police report of accident (if any)

Requirement document for total and permanence disability:
Completed claims form by the beneficiary
A copy of ID card/Passport/Birth Certificate of the Insured;
Document proof of beneficiaries;
Police report of accident (if any)
Recent photo of the Insured;
Medical evidence of the accident as cause of the TPD state provided by licensed hospital or clinic

Hospital allowance (PRAK SOMRAK PET):
Completed claims form by the beneficiary
A copy of ID card/Passport/Birth Certificate of the Insured;

Required Documents for Claim Settlement:
The insurance claim shall be payable provided that the Beneficiary submits the following documents:
A duly completed Claim Form
Loan credit history issued by the Beneficiary Bank
Medical documents related to the treatment
Employer’s statement or hospital discharge letter
Recent loan repayment schedule
Recent photograph of the Insured Person
