Required Documents & Download Form
Before submitting a Death Claim, please ensure all required documents are complete.
Required Documents for Claim:
Claim form filled in by the beneficiary
A copy of ID card/Passport/Birth Certificate of the Insured;
Supporting document of beneficiary (family book, residential book, Loan agreement);
Police report of accident
Medical documents and reports related to treatment;
Recent photo of the Insured;
Death certificate;
Proof of disablement provided by licensed Hospital/Clinics
Requirement document for claim:
Insurance Claim Form completed by the legal beneficiary.
One (1) copy of the Insured’s Identification: ID Card, Passport, or Birth Certificate.
Proof of Beneficiary Status: Family book, Residency book, or Loan Agreement or Loan Repayment Schedule
Police Report confirming the details of the accident or incident.
Medical Records and various treatment reports.
Current Photograph of the Insured.
Evidence of Disability issued by a licensed hospital or clinic.
Proof of the relationship between the Insured and the beneficiary.
